  3. Knives found unclean in storage above 3-vat sink. Must be clean to site and touch. Corrected on site (placed in dishwashing area).

            1. Potentially hazardous foods in 2- door dairy cooler at 48-50°F. Corrected by discard/rapid chill/relocation of foods to operating cooler. Repeat violation.
            2. Dishmachine not reading at proper heat sanitizing temperature; confirmed by thermolabel strips. Must repair machine & sanitize dishes with bleach solution in 3-vat.
            3. Chili in 2-door upright cooler at 49-50°F, in cooling process overnight. Must rapidly/properly chill. Corrected by discard.
            4. Multiple potentially hazardous, ready-to-eat foods in cooler not date-marked with make/open/discard dates. Corrected by discard.
            5. Cutting boards & ladles in clean storage have food/debris/grease stuck to food contact surfaces. Corrected on site.

            1. Gaskets of several cooling units are ripped/detached and door on 3-door upright cooler not staying closed. Must repair. Repeat violation.

                Pink warning tag issued, due to repeat & multiple critical violations. Instructed to attend mandatory "Show-Cause" Hearing with Inspector and Food Division Supervisor on 12/01/2011. Reinspection & food safety training will be conducted.
                Critical Violations Found: 9
                1. Hashbrowns on top, make-table cooler held at 51°F; must maintain at 41°F/below.
                2. Dairy products in cooler held at 48°F; must maintain at 41°F/below.
                3. Condensation in dairy cooler observed leaking from ceiling of unit, down onto foods below; must repair unit & relocate storage of foods.
                4. Sanitizing solution in wiping cloth bucket made to over 200 ppm chlorine; test-strip bleached out. Must maintain 50-100 ppm chlorine.
                5. Employee observed not washing hands between: raw & ready-to-eat food handling, glove changes, dirty to clean duties and handling chemicals to immediate food prep. Repeat violation.
                6. Staff observed handling raw meats & shell eggs and then immediately handling ready-to-eat foods; food is adulterated/contaminated.
                7. Food preparation staff cannot demonstrate knowledge of food safety principles in regards to proper sanitizing , cleaning, food handling & handwashing. Repeat violation.
                8. Multiple food contact surfaces found unclean in clean storage. Repeat violation.
                9. Dishes & food contact surfaces not properly sanitized as dishmachine is not reaching 160°F surface temperature; determined by thermal test-strip & temp. gauge on machine.
                Noncritical Violations Found: 6
                1. Non-food contact surfaces of prep tables, exterior & interior surfaces of cooling equipment found with debris/food build-up.
                2. Interior of ice machine, plastic guard & sides has mold/debris build-up.
                3. Cracked tiles under dishmachine have standing, murky water.
                4. Wiping cloths not stored in sanitizing solution. Must store in solution between uses.
                5. Fluorescent light bulbs in cooling units not shatterproof or shielded.
                6. Food prepped in 3-vat sink that has visible grease/debris/food build-up. Must clean frequently.
